Community Connections

In addition to our work on the Social Prescribing pilot Onside is also working with GP practices in Droitwich and Ombersley on our Community Connections work. This provides an additional resource to the practices and the Social Prescriber in the provision of more in depth support. Where patients need additional input to access services or overcome problems then a referral can be made to Community Connections. A team of volunteer mentors can work on a one to one basis with individuals. The service also helps people overcome loneliness and social isolation by bringing patients together to build social links and friendships.

Jean aged 58 was struggling with social isolation and anxiety and now regularly attends our Community Connections peer support group,

“My family can’t believe this, I don’t go out, my son came with me in the taxi to check where I was going. It’s miraculous. My mentor just gets it. She has succeeded where many others have failed. I’m struggling but with her help I’m here.”
